The resettlement of people with learning disabilities from long-stay hospitals has been under way for a number of years in England. This process has generally been dominated by the exigencies of the available services. A needs-led approach, emphasized in recent community care legislation, in which new services are sought on the basis of the specific requirements of services users, has been used infrequently for this purpose. Compares the process of implementing a needs-led care management approach to resettlement with the implementation of a more traditional service-led approach. It also suggests some important lessons for the application of a needs-led approach to hospital resettlement.

The Multiple Needs Assessment and Care Management Package is a 27 page document which is designed as tool for the assessment of homeless people with mental health problems. It is accompanied by assessment forms contained on a 3.5” computer disc. The pack looks holistically at the presenting needs of an individual. It also enables assessors to: prioritise client needs; action those needs appropriately; and, to develop an effective evaluation process, i.e. working with clients on achievable, realistic goals, with scope for re-negotiation as required. The pack is designed on the assumption that the Assessor is competent in assessment practice and has a good understanding of the parameters that are involved.
Keeping consumers at the centre of planning

Reports on a survey of Community Mental Handicap Teams as part of an evaluation of the All Wales Strategy. Organisational structures of CMHTs are discussed, and a framework for service planning is then outlined, focusing on the co-ordination of service delivery at a case level and on service planning and development. It is argued that whereas case co-ordination may be obtained through a key worker or case manager system, service planning and development are among the strengths of teamwork. Aspects of teamwork are then explored with reference to needs-led services, local community-based provision and consumer participation. Finally, the level of delegated authority and organisational support required to achieve an 'ideal' needs-led team is outlined.

This article introduces service changes impacting on adults with learning disabilities, defines the concepts of learning disability, needs and need assessments, dual diagnosis and mental health needs and considers their service implications. The findings from a national multi-professional survey of services for adults with learning disabilities and mental health needs are summarised and the difficulties of meeting the complex needs of these adults are demonstrated. Recent developments in clinical diagnostic assessment, needs assessment, care planning, outcome monitoring, staff training and service evaluation are discussed, including the possible advantages of the care programme approach (CPA). Recommendations are given for services for adults with learning disabilities and mental health needs and points for discussion with service users and carers are suggested.
