The relationship between challenging behaviour and mental ill-health in people with intellectual disabilities: a review of current theories and evidence

Challenging behaviours and problems of mental ill-health are common amongst people with intellectual disabilities. The article examines conceptual similarities and differences between these conditions, examines the data on comorbidity, and explores possible hypothetical relationships between behavioural and psychiatric disorder in this population. While there is little evidence at present to suggest that many of the challenging behaviours seen in people with intellectual disabilities are underpinned by problems of mental ill-health, only qualified conclusions are possible because of limitations in the quality and scope of existing research. A conceptual model for looking at the risk variables that may contribute to both conditions is suggested, and requirements for future research and current multi-disciplinary practice are outlined.

This article explores the spiritual dimension in mental health care and its relevance to people with learning disabilities. A discussion of the development of a working group to scope the issues in one learning disability service is explored, with practical commentary on the efforts made to gather more information from service providers. The findings from a brief survey are briefly discusses, as well as developments which have ensued to date. The authors conclude that, although spirituality can mean different things to different people, responding to the diverse spiritual needs of service users must take a person-centred and a flexible approach.

The mainstreaming of students with mental health and/or learning support needs means further education college tutors are carrying an increasingly heavy burden of social and emotional care, often in crisis situations. This argues that colleges should do more to support their tutors in their pastoral role.

The paper will describe a formulation-driven approach with reference to a social-cognitive model of mental ill-health in people with learning disabilities. This process is illustrated with a brief case study of an assessment within this model.
Life events as a risk factor for psychological problems in individuals with intellectual disabilities: a critical review

Stressful life events such as bereavement, moving house and changing jobs have repeatedly been implicated as risk factors for mental and physical ill health. Since the 1940s, researchers have demonstrated the negative effects of stressful life events, refined methods of recording such events and investigated the relative impact of different types of event. These investigations have generally not extended to include people with intellectual disabilities. A narrative review of research on life events as they occur to people with intellectual disabilities was conducted and the evidence that life events function as a risk factor for psychological problems critically assessed. Evidence was reviewed for an association between life events and a range of outcome variables, including affective disorders, challenging behaviour, psychosis and psychological problems more generally. The methodology behind the current evidence base was critiqued and a number of methodological advances that would help to strengthen it discussed. There is reasonable evidence that life events are associated with psychological problems, and that there is some tentative evidence that life events play a causal role, although to date, no relationship with psychosis in people with intellectual disabilities has been demonstrated. Life events are likely to be pertinent in clinical work with people with intellectual disabilities.

The organisation provides supported accommodation for people with mental illness, learning disabilities and substance misuse problems in Slough, Reading and Maidenhead. Overcoming the label of learning disability may be an achievement but also triumphing over the obstacles caused by prejudice at being labelled with schizophrenia is highly unusual. This amazing account of triumph over adversity should be an inspiration to other people who find themselves labelled with severe mental illness. The honest and thought provoking way Philip describes his symptoms should enable the layperson to gain an ‘insiders’ insight on what it like to suffer from the active symptoms of Schizophrenia. This book will hopefully challenge the stereotypes surrounding Schizophrenia.
Women with and without intellectual disability and psychiatric disorders: an examination of the literature

This article examines the literature on women with and without intellectual disability and psychiatric disorders, using a gender social model of health. Relevant empirical studies, international literature reviews and policies between 1980 and 2007 were identified from electronic databases, journals and secondary sources. Three areas were examined: psychiatric disorders, their contextual background, and their clinical presentation. There are minimal levels of research into women with intellectual disability and psychiatric disorders. However, this article hypothesizes that women with intellectual disability have higher rates of psychiatric disorders than women without. This may result from greater vulnerability related both to internal factors (`intra': cognitive deficits, poorer communication skills, limited social skills) and to the external world (`inter': lack of opportunities, stigma, poor social support networks). The article argues that such women require gender-sensitive mental health services. However, more empirical evidence is required to support this claim and to inform development and delivery of services.

The UK's policy architecture, goals and values, and its service frameworks, for people with disabilities have been set in the context of the global (United Nations) and European (EU) advances. This article examines the impact of these structures and contexts on people with learning disabilities and mental illness. It explores how identities which cut across recognised categories of disadvantage limit the rights of those with both learning disabilities and mental health problems, and looks at some of the ways in which this problem is being addressed.
