How social care staff can support people with learning disabilities and dementia

This factsheet provides information on how social care staff can support people with learning disabilities and dementia. It covers recognising signs of dementia, sharing information about what a diagnosis of dementia means, and things social care staff can do. It is the 11th in a series of factsheets which show how social care staff can support the health needs of people with learning disabilities. Improving services for people with learning disabilities and dementia: findings from a service evaluation exploring the perspectives of health and social care professionals

Background: This study, conducted as part of a wider service evaluation, explored community learning disability team perspectives on screening, pathways, training, information and supports developed to improve services for people with learning disabilities and dementia. Methods: A focus group was held with health and social care professionals working in community learning disability services. Thematic analysis was used to analyse the data. Results: The dementia screening, pathways and processes had become embedded in practice, leading to a common framework, an efficient, multidisciplinary, proactive approach, earlier detection and diagnosis of dementia and identification of other health needs and issues. This avoided crisis situations supporting people to remain at home longer. Training and information were felt to improve care quality and reduce caregiver anxiety. People with learning disabilities and caregivers were involved to varying extents. External influences impacting on support included the availability, appropriateness, cost and effectiveness of different models of service provision. Conclusions: Service developments have been made as a result of the findings which suggest that dementia pathways and supports improve service provision and outcomes for people with learning disabilities. It is important to develop the evidence base on the effectiveness of different service models for people with learning disabilities and dementia. Future studies need to gather views of people with learning disabilities and carers. (Edited publisher abstract)
Down syndrome health screening: the Fife model

People with Down syndrome have a greater risk of developing a range of health problems, including cardiac problems, thyroid disorders, sensory impairments, reduced muscle tone (hypotonia) and Alzheimer's disease. Despite this increased risk, regular screening is not typically offered to individuals with Down syndrome. A multidisciplinary health screening clinic was set up by a Community Learning Disability Team in Fife to offer regular health screening to people with Down syndrome. The format of the clinic, outcomes after 18 months and future service developments are discussed.

People with Down's syndrome are far more likely to develop dementia than the general population, and at a younger age - the average age of dementia onset is just 52 years, with death likely to occur within just six years . This new resource for family carers, staff and other professionals supporting people with Down syndrome, to help them with practical day to day issues, including: supporting a person to maintain skills and independence as long as possible; treating treatable conditions; understanding and responding appropriately to changes in behaviour, at early, middle and late stages of dementia; providing emotional reassurance for carers; improving the confidence of carers to look after individuals; iIdentifying local supports and resources; and being open to the problems that may arise, and help with tips and solutions to cope.

Alzheimer’s disease and related dementias affects a significant number of adults with intellectual disability (ID), in particular those with Down syndrome. Many affected adults live in small community group homes or with their families. How to provide sound and responsive community care is becoming a challenge for agencies faced with an increasing number of affected adults. This study reports the outcome of a survey of group homes serving adults with ID and dementia, explores the onset, duration and effects of dementia, and speaks to the impact of these on planning for community care of adults with ID. It also examines emerging community care models that provide for “dementia capable” supports and services. Two models, “aging in place” and “in place progression” are examined, as well as “referral out” reactions, with regard to care practices and critical agency decision making.

A number of physical, psychosocial, or ethical issues related to the care of the individual with advanced dementia are reviewed and related to individuals with intellectual disabilities.The author notes that through anticipation and early planning, advanced directives and service planning (which looks to adaptation of services and other care management interventions), can effectively impact care at the end. Areas that need to be addressed include technical information, including a reviewof and, as appropriate, adaptation of general advanced dementia resources, relief, rest, support, reassurance, receipt of on-going information, participation in planning, a sense of humor, and appreciation. Also noted, are the differences experienced because of the presence of paid staff as carers and residence outside of the family home.

This guide is intended for direct care staff who are caring for adults with a learning disability and dementia. Some of these people will be over sixty five but some, mainly with Down's Syndrome will be in their fifties. Staff may be working in a variety of settings where there may have been an emphasis on self determination and independence.

An increasing number of people with Down syndrome are at risk of dementia resulting from Alzheimer's disease. Many reside in community group homes. When they are affected by dementia, the challenge to agencies providing group homes is how to best provide continued housing and provide effective dementia-related care management. In the general population, long term care is typically provided in nursing facilities, often in special care units (SCUs). This study evaluated select factors found in group homes andSCUs to determine which is able to provide a better quality of life for people with Downsyndrome affected by dementia. Results indicate that group homes can provide conditions associated with better quality of life and, additionally, operate with lower staffing costs due to the non-utilization of medical staff.

These conference papers explore the issue of quality in later life, the factors that affect older people's experience and their understanding of what matters to them. Gerontologists have much to contribute both to analysis and the promotion of quality in old age. This contribution continues to develop the means by which older people, including those with significant mental or physical disabilities can make themselves participate in the debates by defining their own experiences and what makes a difference to them.
