Like the rest of the population, people with learning difficulties are living longer and are also increasingly joining the growing numbers of people who have dementia. This trend throws up a range of issues for policy-makers and practitioners. Little attention has been paid to how choice and empowerment, fundamental to the community care reforms, can be made meaningful for individuals with learning difficulties and dementia. This research examined how far 20 people with these dual impairments, living in a range of settings, were involved in making choices and decisions about their own lives, and identified what facilitated or hindered that process.

Choice and control are at the heart of developments for people with learning difficulties. The recent White Paper, Valuing people: A new strategy for learning disability for the 21st century, acknowledges this in its key principles. People with high support needs (who may be labelled as having severe or profound learning difficulties and may also have physical and sensory impairments) may find themselves excluded from these developments. The Community Care (Direct Payments) Act 1996 gave local authorities the power to make cash payments to community care users for the purchase of their own support. Development of direct payments has been slow, particularly for people with learning difficulties. Describes a study which looked in detail at the legal aspects of making cash payments to people with learning difficulties and examined some of the implications for practice, using case studies, in three local authorities.

Government policies across the UK challenge service providers to make sure that people with learning difficulties are central to planning the services that affect them. 'Helping Each Other For A Better World' is a consultancy team of six people with learning difficulties.
