Day services and home care for adults with learning disabilities across the UK

Purpose: This paper compares data from national social care statistics on day services and home care for people with learning disabilities across England, Scotland, Wales and Northern Ireland. Design/methodology/approach: National social care statistics (England, Scotland, Wales and Northern Ireland) reporting the number of adults with learning disabilities accessing day services and home care were reviewed, with data extracted on trends over time and rate of service use. Findings: Regarding day services, despite some variations in definitions, the number of adults with learning disabilities in England, Scotland and Wales (but not Northern Ireland) using building-based day services decreased over time. Data from Scotland also indicate that adults with learning disabilities are spending less time in building-based day services, with alternative day opportunities not wholly compensating for the reduction in building-based day services. Regarding home care, there are broadly similar rates of usage across the four parts of the UK, with the number of adults with learning disabilities using home care now staying static or decreasing. Social implications: Similar policy ambitions across the four parts of the UK have resulted (with the exception of Northern Ireland) in similar trends in access to day services and home care. Day service provision for people with intellectual disabilities: a case study mapping 15-year trends in Ireland

Background: Day services for people with intellectual disabilities are experiencing a global paradigm shift towards innovative person-centred models of care. This study maps changing trends in day service utilization to highlight how policy, emergent patterns and demographic trends influence service delivery. Methods: National intellectual disability data (1998–2013) were analysed using WINPEPI software and mapped using QGIS Geographic Information System. Results: Statistically significant changes indicated fewer people availing of day services as a proportion of the general population; more males; fewer people aged <35; a doubling in person-centred plans; and an emerging urban/rural divide. Day services did not change substantially and often did not reflect demand. Conclusions: Emergent trends can inform future direction of disability services. Government funds should support individualized models, more adaptive to changing trends. National databases need flexibility to respond to policy and user demands. Future research should focus on day service utilization of younger people and the impact of rurality on service availability, utilization, quality and migration. (Publisher abstract)

Statistical information on community care services gathered from health and social care (HSC) trusts. It includes details of contacts with HSC trusts, care packages in effect, meals on wheels services, residential and nursing accommodation and day care registrations. It reports that between 1 April 2016 and 31 March 2017, 28,653 people in the Elderly Care, Learning Disability and Physical and Sensory Disability POC’s were in contact with HSC Trusts. As at 30 June 2017, there were 5,238 residential places available, a 1 per cent increase from the previous year. Of these 59 per cent were in independent residential homes, while 21 per cent were in statutory residential homes and 20 per cent in dual registered nursing homes. (Edited publisher abstract)
Trauma-Informed day services: an initial conceptualization and preliminary assessment

Trauma-informed care (TIC) is a systems-focused philosophy of service delivery based upon principles of choice, collaboration, empowerment, safety and trustworthiness that recognises the pervasive impact of trauma across the human experience. In a grassroots effort, one organization developed an innovative, trauma-informed day program to meet the needs of individuals with intellectual and developmental disabilities (IDD) who were recently deinstitutionalized. The present study is intended to provide an initial conceptualization and preliminary assessment of TIC within IDD services in order to understand its impact among individuals and staff. The study sought to answer the following questions: Has the program's culture been stable over time? How have individuals’ behaviours changed over time? What have been the experiences of the program's staff members with TIC? Through a mixed methods design, secondary data analysis and semi structured staff interviews were used to assess the impact of TIC. Findings revealed an initial strong presence of choice, collaboration, empowerment, safety, and trustworthiness within the program's culture, with non significant changes at follow-up. Significant reductions were noted in overall challenging behaviour, aggression, and PRN medication usage; while non significant changes were noted in physical interventions with the exception of “other” interventions (i.e., least restrictive) which significantly increased. Three major categories emerged from the qualitative data (making a difference, recognising progress and compromising factors), illuminating staff satisfaction with work experiences, individuals’ progress, and factors that challenged fidelity to TIC. The study provides a preliminary conceptualization and evidence for the efficacy of the integration of TIC into IDD services despite methodological limitations and concerns regarding the use of physical interventions. The state of Shared Lives in England: report 2017

This report draws a survey of Shared Lives schemes in England to provide an analysis of services across England for the period 2015/16. The report provides figures on the numbers of people who use Shared Lives services, the type of arrangements they live in (live-in, short break and day support), the regional breakdown of services, the number and characteristics of carers, and staffing levels. The report finds that the Shared Lives sector has grown by 5 per cent over the past year, with approximately 11880 people being supported in Shared Lives arrangements. People with learning disabilities remained the primary users of the service, making up 71 percent of all users. This is despite a small reduction in the number of people with learning disabilities accessing the service in the previous year. The next largest group getting help from Shared Lives were people with mental health problems, who made up 8 per cent of users. Short case studies are included to illustrate the benefits of Shared Lives schemes. It ends with key learning from the past year and identifies some of the key factors and barriers to the successful expansion of Shared Lives. (Edited publisher abstract)
