A workbook for panel members to record their findings during a Care and Treatment Review (CTR) for adults with autism or learning disabilities in the community. The workbook is based on the key lines of enquiry (KLOE) and shows what sort of questions to ask and where to find evidence. It also provides space to record findings and to record what you think if the person’s quality of care overall for each KLOE. The KLOEs, are: Does the person need to be in hospital?; Is the person receiving the right care and treatment?; Is the person involved in their care and treatment?; Are the person’s health needs known and met?; Is the use of any medicine appropriate and safe?; Is there a clear, safe and proportionate approach to the way risk is assessed or managed?; Is there active planning for the future and for discharge?; and Are family and carers being listened to and involved? It will also help with deciding what goes into the CTR report. It is one of a series of 6 workbooks, which each cover different types of CTR and Care, Education and Treatment Review panels. This report is issued as best practice guidance to councils with social services responsibilities and health bodies. It is not mandatory and no extra resources will be provided for its implementation. Councils and health bodies should take it into account in setting their own priorities and policies. It will also be useful to people using services, their families and representatives, staff and those responsible for commissioning of local services as a statement of best practice. It is an updated version of the guidance originally produced by Professor Mansell and his project team in 1993.

This thematic research review brings together NIHR-funded research on health and social care services for people with learning disabilities and their families. It features 23 recent studies and includes summaries of findings. The review covers the themes: understanding learning disability and services; identifying health risks; keeping well in the community; staying well and safe in hospital and preventing avoidable deaths; services supporting positive behaviour; and conducting research together. It also highlights studies that show examples of good practice in involving people with lived experience of learning disabilities in doing research. It also includes the views of people with lived experience around the issues covered by the research. The research finds that more work is needed to improve care for people with learning disabilities, who experience poorer health and die earlier than the general population. Many of these deaths could be prevented by public health interventions or better access to high quality care. It includes important findings for those who commission, deliver, work in and use these services. (Edited publisher abstract)
The extension of a set of needs-led mental health clusters to accommodate people accessing UK intellectual disability health services

Background: A development of a needs-led mental health classification system based on the Health of the National Outcome Scales (HoNOS) has previously been developed. Aims: To extend the needs-based mental health (MH) clusters to accommodate the additional needs of people accessing UK intellectual disabilities health services. Method: Hierarchical cluster analysis was performed on assessment data from 18 National Health Service (NHS) provider organisations. The statistical results were clinically shaped through multi-disciplinary workshops. The resulting clusters were combined with six independently rated measures for a second data collection exercise. Based on these data, refinements were made before performing internal and external validity checks.Results: Eight additional clusters for people with health needs associated with their intellectual disabilities were produced. Three described primarily physical health (PH) needs, four described needs arising from behaviours which challenged (with/without autism) whilst one described people with generally low needs. Together, these covered 83.4% of cases with only a 10% overlap. The clusters were replicable and had clinical utility and validity. Conclusions: It was possible to extend the needs-led mental health classification system to capture the additional needs of people accessing UK intellectual disability services.
