Comments on the paper “Early intervention for children with learning disabilities: making use of what we know” by Nick Gore, Richard Hastings and Serena Brady. lt places the arguments made by Gore et al. in a broader scientific and policy context. It finds the arguments to support increased investment in early intervention and prevention presented to be fully consistent with the broader scientific literature on prevention and the required future direction of English health policy. This resource is written to help with the selection and evaluation of outcomes of intervention for people with learning disabilities whose behaviour presents challenges for services. The authors cover a range of potential outcomes, enabling selection of those which are most relevant in order to evaluate progress towards achieving those outcomes. They look at both the personal and social consequences of the person's challenging behaviour and the effect of interventions on these quality of life factors, rather than simply focusing on attempting to reduce the challenging behaviour itself. The book is designed to act as an addition to existing assessment and intervention processes and documentation, and can be adapted to suit individual circumstances. Sections include: positive goals and positive behavioural support (PBS); development of positive goals; and how to use positive goals. It is aimed at carers, clinical psychologists, psychiatrists, behaviour therapists, community learning disability nurses, social workers, care managers, members of community learning disability teams and members of challenging behaviour teams. The editors’ aim when compiling this book was to provide a resource that will support the training of clinical psychologists and other professionals in their work with people with intellectual disabilities. It is pitched to cover the “middle ground” between a "how to" manual and an academic review of the literature. In depth coverage is provided on all aspects of clinical practice, from key basic concepts and assessment to evidence–based intervention techniques. The book is in three main sections: the first sets the scene looking at issues that underpin services for people with intellectual disabilities. The second part addresses a wide range of clinical issues, while the third focuses more specifically on issues related to clinical practice with specific client groups such as people with challenging behaviour, older people and parents with learning disabilities themselves. All chapters in this new edition have been fully updated with the latest evidence. An evaluation of specialized community-based residential supports for people with challenging behaviour

This study explored the effects of relocation from institutional to specialized community-based residential provision for six men with mild to moderate intellectual disabilities and challenging behaviour and for a comparison group of six men with mild to moderate intellectual disabilities and challenging behaviour who were already living in specialized community based residential provision. Relocation was associated with: (1) a significant increase in participants’ domestic activity skills; (2) a decrease in the observed occurrence of some problem behaviours; (3) an increase in quality of life; and (4) an increase in engagement and staff contact. The community group, however, also showed some improvements over time, most notably in their levels of engagement in meaningful activity and staff contact. The implications of these results for service provision and suggestions for future research are discussed.
