Evidence confirms that people with learning disabilities’ uptake of screening services is lower and that they have poor oral health when compared to the general population. Good oral health is linked to good general health and a number of systemic diseases have oral manifestations. Improvements in oral health care can be obtained by encouraging carers to regularly examine the mouth of the person they are caring for. Completing an oral assessment will help to identify any changes in the mouth. This can aid diagnosis and treatment if reported to the dentist at an early stage. vThe guidelines are based on published evidence and follow the format of other clinical guidelines developed by the Faculty of Dental Surgery The Royal College of Surgeons of England and those from the British Society of Disability and Oral Health. Information was gathered from consultation groups of people with mild to moderate learning disabilities. Postal questionnaires were also used.
Support for people with learning difficulties in residential settings who develop dementia

What happens when someone with learning difficulties develops dementia? This study explored the experience of 18 people with learning difficulties living in small-scale, community-based residential homes. It also looked at the knowledge and experience of service providers, staff, fellow residents and relatives. Lack of planning led to ad hoc arrangements that often failed to meet the needs of people with dementia, fellow residents and staff. Lack of coherent strategies and resources resulted in people being moved inappropriately to nursing homes for older people. When people did move to another setting, residential home staff continued to play a vital role in maintaining basic care for the individual. There was a lack of coherent, consistent practice in relation to diagnosis and follow-up care. Where baseline assessments were established there was a marked improvement in early diagnosis. The use of consistent, good quality, practice-based training was critical in determining the type of care given and the likelihood of someone remaining 'at home'. Staff often provided unpaid or underpaid support. They played down the increased complexity of individual care needs because they feared requesting additional help might result in the person being moved. Waking night staff were essential not only for dealing with night-time disturbances but also for providing one-to-one support. This also had a positive impact on fellow residents' ability to accept and cope with the person with dementia. In particular, staff in all settings struggled with pain management and helping people to eat well. Dementia-friendly built environments were rare. But in some cases staff had been imaginative and resourceful, demonstrating that such changes do not have to be expensive.
Working with families, friends and carers: a framework for adult social care employers

This framework, developed in partnership with the Avenues Group, sets out good practice guidelines about the skills and knowledge adult social care staff need to work effectively with the families and carers of people who need care and support. The framework covers four areas: establishing positive relationships with families, friends and carers; recognising the importance of family relationships and your role in this; maintaining appropriate communication with families; and offering support to people who need care and support to maintain and manage family relationships. The framework can be used to help organisations design or commissioning training. It can also be used to review how services and staff are working with families and carers and identify what needs to improve. It is based on research carried out by Avenues Group with people who need care and support and their families. Links to a sample training session designed by the Avenues Group is included. Investing in later life: a toolkit for social services providing care for older people

This toolkit provides recommendations and examples of innovative practice for professionals working in social services providing care for older people. It is based on desktop literature research and the result of the European Social Network’s (ESN) work on ageing and care between 2014 and 2016. Chapters cover knowledge and best practice around initiatives to promote active ageing and independence, carers support, improving the coordination of health and social care services, and support for people with complex needs and disabilities as they grow older. Chapter one focuses on the promotion of active ageing and discusses key developments such as supporting volunteering, prevention and early intervention, and rehabilitation. Chapter two highlights the challenges in recruiting sufficient qualified social care staff, particularly in the provision of long-term care and looks at how to better support informal carers. Chapter three discusses cooperation and integration opportunities in older people’s services. It analyses the challenges of integrated services at three different levels: macro-level, meso-level and micro-level. The final chapter explores how people with multiple conditions can be supported most effectively in a person-centred way, including people living with dementia and people with learning disabilities. The keys to life: report of the Care Inspectorate's inspection focus area 2014-2016

This report presents the results of thematic inspections of services for adults with learning disabilities in Scotland, which were carried out to examine the quality of services and the extent to which the key principles of The Keys to Life policy were being met. It also looks at the extent to which the learning from the Department of Health review into Winterbourne View Hospital in Gloucestershire is informing practice in Scotland. The inspection included care homes, care at home services, housing support services and d combined care at home/housing support services for people with a learning disability. The report examines the findings in relation to outcomes for people who use these services, and their carers, aligning these to the four overarching strategic outcomes of The Keys to Life: a healthy life; choice and control; independence; and active citizenship. The inspections found that over 93 per cent of the services were providing good, very good, or excellent care. They also identified a high-level awareness about The Keys to Life strategy. Where services were good, this related to the implementation of person-led care practices which promoted choice and protected the rights of those using services. Areas for improvement identified included: some care managers reporting difficulties in accessing the right healthcare for the people they support; improvements in the way care was planned and delivered; and providing activities that were better focused on people’s individual choices. Working lives: the role of day centres in supporting people with learning disabilities into employment

The qualitative research took place in nine Local Authorities across England, to obtain the perspectives of users of day services, their carers, managers and staff of days services, and managers of employment-related providers, both within the Local Authority and independent of it. Key findings include: the majority of people interviewed who worked were paid. Most people earned only the minimum wage or just above; most people worked for less than five hours per week, some worked five to 15 hours, and a small minority worked more than 16 hours; carers were more critical of day services than were people with learning disabilities; carers views on employment varied widely, but many carers were powerful advocates for employment. Many carers we talked to valued work highly and felt that being employed was beneficial to their relative; day centres varied greatly in the emphasis they placed on achieving paid employment outcomes. Some focused on employment and understood good practice in the area, while other were imprecise about how their activities helped people to get jobs; it is easier for people to find and stay in work if they receive specifically targeted support in which employment of their choice is assumed to be a goal; evidence suggests that a comprehensive range of work exploration, work placement and support services are required to help a greater number of people into paid jobs; the reason people work few hours is mainly due to the perceived and potentially real loss of earnings due to Income Support rules which acts as a disincentive, rather than due to limits deriving from health and support needs.

This report is written for service providers, families, friends, carers and other who support people with learning difficulties who have high support needs to make choices. It uses real life stories to challenge the idea that people with high support needs cannot make their own decisions. It discusses current practice and law, suggesting both are often limited in vision and technique, and offers a model of supported decision making as a solution. It is accompanied by a workbook 'It's your choice' written in plain English, with pictures, to tell people with learning difficulties about making decisions.
