Care management is a new job requiring people who, as well as sharing a value base, combine imagination, tenacity, realism and financial acumen. So far there is no accredited training for this work. In this article the author describes how packages were developed for three individuals in South Cambridgeshire.

Micro-organization is currently fragmented in services for people with learning disabilities. Care management, person-centred planning (PCP) and direct payments have developed through separate policy strands, with tasks and agency responsibilities blurred. A wide diversity of care management arrangements currently operate, with the relationship between care management, PCP and direct payments imprecisely defined. PCP and direct payments have also been variably implemented. This paper argues for a new ‘person-centred case management’, with these different devices better integrated and decision-making and action more person-centred. Drawing on practice experience from the original British case management experiments, the new ‘case’ management would be centred on the needs and wants of individuals, be conducted independently from assessment, operate outside the public sector and be able to access personal budgets. It would consequently have the capacity to further de-institutionalize services and support and transfer more control to people with learning disabilities.

Development of the Nutrition and Swallowing Checklist, a screening tool for nutrition risk and swallowing risk in people with intellectual disability

Nutritional problems in this population group are generally under-recognised by carers and professionals alike. Dysphagia and its consequences, including nutritional consequences, are also poorly recognised. In 1997 the New SouthWales Department of Community Services decided to develop a systematic approach to improving recognition, assessment, and management of nutrition problems and of dysphagia in its clients with intellectual disability. One of the strategies of the project was to develop a screening tool to improve awareness by care staff of nutrition risks and swallowing risks in the clients for whom they care. The Nutrition and Swallowing Checklist encourages carer involvement in identifying these risks and an interdisciplinary approach to the assessment and management of nutrition and dysphagia. This paper reports on the need for the screening tool and the development process up to the point of introduction into services.
An investigation of the effects of bereavement on mental health and challenging behaviour in adults with learning disability

Reports on a study to investigate the impact of bereavement on people with a learning disability. Twenty adults with learning disabilities who had experienced the death of a primary caregiver in the previous 2 years were compared with a matched control group on measures of psychiatric disturbance and challenging behaviour. With regard to psychiatric disturbance, significant differences were found for 'neurotic disorder' and for 'organic condition' among the bereaved group on the Psychiatric Assessment Schedule for Adults with a Developmental Disability (PAS-ADD). No difference was found for 'psychotic disorder' on the PAS-ADD. As regards challenging behaviour, significant differences were found for 'irritability', 'lethargy' and 'hyperactivity' among the bereaved group on the Aberrant Behaviour Checklist (ABC). No differences were found for 'stereotypy' or 'inappropriate speech on the ABC. Outlines the clinical implications of these findings and presents recommendations supporting people with learning disabilities who experience bereavement.
