The impact of the Supporting People programme on adults with learning disabilities

The Supporting People programme, which began on 1 April 2003, was designed to separate out the costs of bricks-and-mortar housing (which, where needed, would continue to be paid through Housing Benefit) from the costs of the support necessary to enable vulnerable adults to attain or maintain independent tenancies. For people with learning disabilities, this new funding mechanism appeared to offer a much-needed opportunity for some of the changes set out in the 2001 Valuing People White Paper to be made a reality. This research project set out to examine how local Supporting People teams were interpreting national guidelines in relation to the provision of housing-related support and to explore the impact that this was having on people with learning disabilities. It also hoped to discover the extent to which the Valuing People core aims – of promoting rights, choice, independence and control in the lives of people with learning disabilities – were being supported by this new programme.
The impact of Irish policy and legislation on how adults with learning disabilities make choices

This paper reflects the impact of policy and legislation in the context of how adults with learning disabilities make choices. Following an overview of policies which have improved choice for people with learning disability in the United Kingdom, this paper reviews ‘choice’ in current Irish policy and legislation. This paper, while making comparisons with capacity legislation in the United Kingdom, presents a discussion on the perceived impact of the Assisted Decision-Making (Capacity) Bill (2013) based on submissions from the Citizens Information Board (2013a), Mental Health Reform (2014), and Quinn (2013). Further choice making challenges such as direct payments and information sharing which confront adults with learning disabilities are highlighted and overall conclusions are drawn. Realising the vision of communication inclusion

Purpose: The purpose of this paper is to provide a commentary on Clare Mander’s paper entitled “An investigation of the delivery of health-related accessible information for adults with learning disabilities”. The commentary discusses issues identified by the author in the field of accessible information and, more generally, inclusive communication practice. Design/methodology/approach: This commentary reviews factors which influence the communication accessibility of services and the policy, legislative and economic case for mainstreaming inclusive communication approaches. It highlights diversity in the definition of inclusive communication and the scope of its necessary implementation before proposing an approach to establishing consensus on the way forward. The commentary suggests how the task of mainstreaming inclusive communication throughout public services could be addressed, reflects on progress to date and proposes next steps for the growing numbers of those with an interest. Findings: Many factors influence communication accessibility. The communication attitude, knowledge, skills and resources individual providers and recipients bring to each interaction are as important as the whole organisation’s approach towards and investment in inclusive communication approaches. Legislation, policy, demographics and reducing budgets increase the requirement to mainstream inclusive communication approaches. Achieving this objective is an enormous task. Originality/value: Reflecting on all the drivers and influences on the objective of achieving mainstreamed quality inclusive communication practice throughout public services, the author offers potential suggestions for starting to address this enormous but worthwhile task with a view to generating discussion and collaboration among those with an interest. A review of the literature relating to psychological interventions and people with intellectual disabilities: Issues for research, policy, education and clinical practice

There is a need for a long-term focus on the development of policy, research education and clinical practice relating to psychological therapies for adults with learning disabilities. This study identified and analysed the published evidence base and wider literature in relation to psychological interventions. A total of 25 English language papers were reviewed. Findings indicated that the evidence base regarding psychological interventions, while limited, were growing, and if the therapeutic approaches were modified and adapted to meet the distinct needs of people with intellectual disabilities these may be life enhancing. The lack of access to psychotherapies for people with intellectual disabilities has led to their exclusion from mainstream research, thereby limiting the evidence base on effective interventions and treatment approaches. The authors concluded that this has significant implications and is an area requiring both strategic and local development in the future.
It’s about time: An overview of short break (respite care) planning and provision in Scotland

This research was carried out for Shared Care Scotland by Reid Howie Associates to look at the planning and provision of short break care in adult and older people’s services. The report presents the findings of research carried out between April and August 2010 to: improve knowledge and understanding of the landscape of respite care and short break provision for adults in Scotland; identify good practice in the planning, commissioning and design of respite care and short break services; and better understand the challenges and identify solutions to moving towards more flexible, personalised respite care and short break services for adults. The research focused on the views of stakeholders in the public sector, although a number of carers’ and other organisations also contributed views.

The Proposed Statement is a comprehensive document, covering the lives of people with a learning disability and outlines the desirable outcomes in different areas of their lives. Areas such as community living and employment are covered in detail with a clear statement of what a person with learning disabilities should have and a list of detailed recommendations of how that aim can be fulfilled. The document is both a recognition that things need to improve for people with learning disabilities, and guidance of how that can be done. It builds on the All Wales Mental Handicap Strategy.

Improving outcomes, enhancing care: assistive technologies and the case for a sector deal for the learning disability sector

This report outlines the key arguments for a Sector Deal for the adult learning disability sector to realise the full potential of technology, boost investment and transform the way care is delivered. It highlights the challenges faced by a social care sector in financial crisis at a time when demand is growing. It argues that closer collaboration between assistive technology companies and social care providers would promote investment and innovation in the learning disability sector through increasing and novel use of assistive technologies. The report includes individual case studies to show how assistive technology can make a positive difference to people with learning disabilities living in a variety of living environments. Through managing risks such as fires or falling, aiding communication and helping to deliver greater privacy or dignity, technology can enable people to have more control over the way they live. Smiling matters: oral health care in care homes

This review draws on one hundred inspections of care homes in England to examine oral health in care homes, with specific reference to the NICE guideline NG 48 on oral health for adults in care homes. It includes direct quotations from managers, staff and people using services and their relatives. It also includes good practice examples to highlight the benefits of good oral care for older people, people with learning disabilities and people living with dementia. The review found that staff awareness of the NICE guideline recommendations was low, over half of care homes visited had no policy to promote and protect people’s oral health, and nearly half of care homes were not providing staff training to support people’s daily oral healthcare. The review also identified challenges such as: a lack of dentists who were able or willing to visit care homes and local dentists not accepting new patients and the length of time it took to get an appointment with an NHS dentist. The report recommends mandatory staff training in oral care, oral health check-ups for all residents upon admission, better signposting to local dental services and awareness raising. Core human rights concepts in Irish health and housing policy documents: in search of equity for people with ID

If specific groups and their needs are excluded from policy, this exclusion may extend to service delivery and add to the inequities that already exist in health and housing access for vulnerable groups. The extent to which Irish housing/de‐institutionalisation and health policies include core human rights concepts has not been examined previously. This article examines the extent to which the core human rights concepts for people with ID are represented in health and housing policies. EquiFrame was the analytical tool for examining six health and housing policies. EquiFrame analyses policy content on commitment to core concepts of human rights and inclusion of vulnerable groups. Health and housing policies in Ireland, despite outlining a commitment to reducing health inequities, were not found to have a commitment to the core human rights concepts in relation to the specific needs of people with disabilities and people with an intellectual disability. Equity means inclusion of all people and includes recognition, acknowledgement, and monitoring of the specific needs and challenges faced by particular vulnerable population groups, and that these needs are being addressed through policy administered by federal oversight agencies. An implication of the findings is that the EquiFrame tool previously used in the health sector is also useful in considering housing policy. Furthermore, equity concerns need to be examined closely for people with ID because including all vulnerable groups in one aggregated category has been shown to conceal the specific needs and challenges that need to be addressed for specific vulnerable groups.
