Highlights the progress made in end of life care and services in Wales and identifies areas for future improvement. In particular, the report looks at the progress made in implementing the actions set out in the End of Life Delivery Plan and how this is reflected through outcome and assurance measures. They show that: direct feedback from patients and their families about specialist palliative care services in Wales has shown over the past year that these services make an enormous difference to people’s quality of life in their closing days; 80 per cent of specialist palliative care urgent referrals were assessed within the agreed timescale; 93 per cent of the responses to the iWantGreatCare patients’ evaluation of their experience of specialist palliative care were positive; the percentage of people dying in hospital has fallen from 62.6 per cent in 2008-09 to 56.2 per cent in 2014-15; there has been a 15.7 per cent increase in the number of people registered on a primary care palliative register over the last 12 months; there has been a year on year increase in the number of people participating in the National Institute of Social Care Health Research palliative care studies since 2008. There are however a number of areas where further progress is required or where new issues need to be addressed, including: more consistent early identification of patients; allowing more people to receive care and die in the place of their choice; ensuring that there are sufficient nursing and care home places available; reducing unnecessary emergency and hospital admissions; and ensuring that people are better prepared for their deaths. Sets out the Welsh Government’s expectations of the NHS in Wales in delivering high quality end of life care, regardless of diagnosis, circumstance or place of residence and provides a framework for action by local health boards, NHS trusts and third sector partners. The plan is split into six areas: supporting living and dying well; detecting and identifying patients early; delivering fast effective care; reducing the distress of terminal illness for patients and their families; improving information; targeting research. The document sets out delivery aspirations, specific priorities for 2013-2016, responsibility to develop and deliver actions, and population outcome indicators and NHS performance measures. Draft national framework for the delivery of bereavement care in Wales

This consultation seeks views on the Draft National Framework for the Delivery of Bereavement Services in Wales. The framework seeks to set out how to respond to those who are facing, or have experienced, a bereavement in Wales. Bereavement is experienced in relation to the death of a person wherever and whenever it happens. For the purpose of this framework it includes pre-bereavement (a grief reaction before someone dies, sometimes referred to as anticipatory grief) and includes bereavement experience of anyone, whatever their relationship to the person who dies. It includes bereavement due to the death of any person, including those who die before birth. Aimed to those involved and providing bereavement services, the framework includes: implementation of the bereavement standards; implementation of clear referral pathways; introduction of quality standards for the design and delivery of national training programmes. Brings together data and information on how services for those nearing the end of life are performing at a local and national level in Wales. It highlights the progress being made and identifies areas for future improvement. The extent to which key quality assurance measures are met is examined. These include: reducing number of deaths within 3 days of an emergency admission; reducing terminal admissions that are 8 days or longer; ensure that the number of places in a care or nursing home is in line with the needs of the population; advanced care planning; increase the number of patients referred to specialist palliative care, who have had a centrally recorded completed assessment; patient experience; and assessments for palliative urgent care referrals. The report highlights some important priorities that must be addressed at a national level over the next 12 months. These include: encouraging more people to make a will and share their final wishes with family and friends; supporting GPs to ensure that patients are being identified earlier as being in their last year of life; ensuring that systems are in place that supports more people to be cared for and to die in the place of their choice; ongoing implementation of the Welsh integrated care priorities guidance; and further development of iWantGreatCare in Wales.
