This report sets out recommendations regarding the Liverpool Care Pathway (LCP) and end of life care following an independent review of the LCP chaired by Baroness Julia Neuberger. The recommendations include phasing out the LCP and and replacing it with an individual end of life care plan. The Review panel recognised that, when applied correctly, the Liverpool Care Pathway does help patients have a dignified and pain-free death. While the Review panel's members support the principles underpinning it, they heard of too many cases where the LCP was simply being used as a ‘tick box’ exercise, its users failing to take account of a patient’s individual needs.
