There is a concern that, at a time of scarce resources, people will present with trivial disorders, and use resources needed for those with more disabling conditions. This Australian study recruited participants from the community for an internet-based treatment for social phobia. Applications were received from 789 people, and 205 were rejected because of severe depression or suicidal thoughts.

This Australian study recruited participants from the community for an internet-based treatment for social phobia. Applications were received from 789 people, and 205 were rejected because of severe depression or suicidal thoughts. Many were excluded because they had another disorder or were in treatment. Some dropped out, only 7 were subthreshold cases and 291 people with social phobia were treated. Despite easy access to clinicians, this study found a high level of unmet need for serious mental disorders.

Depression remits and recurs, but among what proportion of the population? Retrospective surveys report the lifetime risk to be around 10%. A modelling study and two prospective studies concur that close to half the population can expect one or more episodes of depression in their lifetime.

Major depression was the fourth most important determinant of the burden of human disease in 1990 and is expected to rank second in the world by 2020. It is the principal cause of disability in developed countries and the central question is why the burden of depression is not receding at the same rate as the burdens of infectious and perinatal disorders. This is a situation in which effective prevention can be critical. In population surveys, one-third of people who have met criteria for major depression in their lifetimes report that the first attack occurred before the age of 21. The problem is too important to be restricted to the traditional method of face-to-face intervention, but knowing that cognitive therapy is a cost-effective method of preventing depression in high-risk groups
