The taboo of senior sexuality: enhancing staff confidence

Residents and staff can experience barriers in dealing with sexuality and intimate relationships. This article offers some frameworks for reflection and suggestions on how problems can be overcome. Based on the Royal College of Nursing guidance, the article offers ideas for staff to assess their confidence when broaching issues of sexuality with residents, and dealing with issues of sexuality and relationships. In conclusion, managers should work proactively in developing environments, policies and care practice that support the workforce to promote the sexual rights and choices of residents.

All individuals have the right to sexual expression, yet for people with dementia, these rights are often ignored, denied or stigmatised. Supporting people’s rights to sexual expression and intimate relationships in care homes can raise complex legal, ethical and practical issues. There is potential conflict for staff between promoting residents’ autonomy, choice and rights to sexual expression, and protecting those who are vulnerable, particularly if they lack mental capacity to consent. Staff must work within the law, but there is no single way of dealing with dilemmas – every circumstance is different and situations can frequently change. In this article, the author offers guidance for staff in addressing issues of sexuality in care homes.
Neglect of older people in formal care settings part two: new perspectives on investigation and factors determining whether neglect has taken place

Different approaches and levels of health-focused investigations can be used to achieve different outcomes. Investigations need to be appropriate for, and proportionate to, the concern identified, and should be carried out by people with the skills to look as widely and objectively as possible. This paper categorises different types of health-focused safeguarding investigations and offers suggestions, particularly relevant to nurse investigators, on how an investigation can be approached. Suggestions are offered on how to conduct an investigation, where to find information, how to conduct interviews, writing a report and giving a professional opinion. Criteria for determining whether neglect has taken place in a formal care setting are offered, alongside examples of how these have been applied in practice. The paper concludes with consideration of actions that can be taken following an investigation and some reflections on the experience of professions involved in safeguarding.

This guide aims to support best practice and help nurses and care staff work effectively with issues of sexuality, intimate relationships and sexual expression, particularly for older people living in care homes. As well as raising awareness of issues which can impact on the intimate relationships and sexual activity of older care home residents, the guide contains: legal and professional frameworks within which nursing and caring practice takes place; considerations for policies that address sexuality and sexual health needs in care homes; guidance on principles of good practice, including care environments, organisational systems and care practices; ideas on how to identify barriers to expression of sexuality and work towards removing these in practice; suggestions on how to broach issues concerning sexuality, intimate relationships and sex; guidance on dealing with situations where sexuality expression is seen as a concern; and suggestions on how staff can develop their own confidence and competence in dealing with sexuality issues. It also included case examples, which can be used to highlight issues for discussion and additional resources to support practice. (Edited publisher abstract)

Good leadership is key to preventing the abuse of older people. In the final part of their series on safeguarding the authors ask what it takes to be a successful leader. With specific relevance to care homes supporting people with dementia this article offers ideas for motivating good practice and gives some tips on how leaders can reduce the risk of neglect and organisational abuse.
