Consumer health organisations (CHOs) - broadly defined as non-profit or voluntary sector organisations - offer a resource for supporting self-management and self-care for those with chronic illness. In order to investigate patterns of usage of diabetes and arthritis CHOs a cross-sectional computer-assisted telephone interview survey was completed by 279 people who had made contact with one of four consumer health organisations (CHOs) in Queensland, Australia, between July and August 2006. People contacted CHOs primarily to obtain further information about their condition or to access services or products. Most believed CHOs offered useful information and almost half reported that they had started exercising or changed diet following contact. More than two-thirds of diabetes contacts had been directed to the organisation by a health professional, compared with less than one-third of those with arthritis. The study concludes that people who contact CHOs report benefits and health actions conducive to better self-management. The integration of CHOs within the wider health system, as in the case of the diabetes CHO in this study, is likely to facilitate contact.

Depression can affect people with a range of long term conditions, including musculoskeletal conditions, cancer, diabetes, and heart disease. People with long term conditions are two or three times more likely to experience depression and estimates suggest that 20% of people with long term conditions have depression. The prognosis for people with long term conditions is poorer care, poorer outcomes, a poorer quality of life, and a substantial cost to the NHS. It is estimated that in a range of long term conditions depression can increase healthcare costs by 33% to 169%. Depression can also increase the risk of death for some people with some long term conditions. The success of chronic illness, self care interventions is often dependent on the patient’s personality and this study tries to measure the effects of personality factors on patients’ self-assessments of health. Specifically looking at effects of four of the ‘five factor model’, personality traits of high neuroticism, low conscientiousness, extraversion, and agreeableness, (also grouped as the ‘distressed personality profile’) on ‘the homing in on health’ illness self care intervention on physical and mental health. The study recruited 384 participants (40 years plus) from 12 offices of a university-affiliated primary care network in Northern California, who had one or more of the following, arthritis, asthma, chronic obstructive pulmonary disease, congestive heart failure, depression, and/or diabetes mellitus. Regression analysis examined effects of NEO-five factor inventory scores on mental and physical component summary scores (MCS-36 and PCS-36, measures of mental and physical health respectively) at baseline, 2, 4, and 6 weeks, 6 months and 1 year, adjusting for age, gender and study group. The MCS-36 scores were worse for participants with ‘distressed personality profile’, relative to others, with high neuroticism the most relevant, but the scores did improve with time. There were no personality or intervention effects on PCS-36 scores. The authors call for measures of personality to be introduced as standard to help identify individuals more likely to benefit from self-care interventions.
