Background: The Francis report highlights perceptions of care that are affected by different factors including ward structures. Aims: To assess patient and staff perceptions of psychiatric in-patient wards over time. Method: Patient and staff perceptions of in-patient psychiatric wards were assessed over 18 months. The authors also investigated whether the type of ward or service structure affected these perceptions. They included triage and routine care. The goal was to include at least 50% of eligible patients and staff. Results: The most dramatic change was a significant deterioration in all experiences over the course of the study. Systems of care or specific wards did not affect patient experience but staff were more dissatisfied in the triage system. Conclusions: This is the first report of deterioration in perceptions of the therapeutic in-patient environment that has been captured in a rigorous way. It may reflect contemporaneous experiences across the National Health Service of budget reductions and increased throughput. The ward systems the authors investigated did not improve patient experience and triage may have been detrimental to staff. (Edited publisher abstract)

The terrorist attacks in the US on September 11, 2001 stimulated an unprecedented rapid response by the social and health research communities into the aftermath. This article summarizes the findings of the major studies that assessed various types of “psychological distress,” and identifies some of the important gaps that remain in our understanding of the nature and etiology of human distress following purposeful, unpredictable mass violence.

The quality of care in nursing homes remains far from optimal. Excessive reliance on a medical model of care fails to address residents' desire to be treated with kindness, courtesy and consideration. In addition, the issue of improving quality of care is linked to addressing growing concerns about nursing home staffing. One solution is for government to play a role in catalyzing a change in the organizational culture of nursing homes to address the concerns that both residents and staff have with the existing culture. A number of states are providing grants to nursing homes to implement culture change. This has included New York State which has emphasized culture change in its Dementia Grant Programme. The negative effects of slavery have been theoretically linked to contemporary problems faced by African Americans, such as family instability, low achievement motivation, and high rates of juvenile delinquency and youth violence. Combining historical, sociological, and psychological materials, the current analysis argues that Blacks exited slavery with the necessary social capital, inclusive of proactive family attitudes and patterns as well as high achievement motivation, for rapid acculturation into mainstream America. Shifting to the present, it is shown that the co-existence of high Black crime rates and Black cultural integrity are not contradictory, especially when systemic forces neutralize or undermine the ameliorative potential of Black culture.
