The introduction of internal or 'quasi' markets in welfare as enacted through the NHS and Community Care Act 1990 has been interpreted as the work of a government committed to market ideology. The reforms have resulted in decentralisation of decision-making and, in most cases, the introduction of competition in provision. In health this has meant the splitting of health authorities into purchasing and providing units and the introduction of GP fundholders, with parallel changes in community care including the distribution of care purchasing budgets to individual care managers on behalf of clients. This article discusses the impact of the reforms on local Mind groups as providers of mental health services within the voluntary sector, and the limitations of the market model for describing issues of current relevance to such organisations.
