To improve quality leaders must focus on relationships that facilitate change. This article describes Generations Q, a project from the Health Foundation which has taken a small group of NHS managers, clinicians and patient advocates to developed their quality improvement skills. The leadership course focuses on integrating the technical skills of quality improvement with an understanding of how to manage relationships. Participants give their views of attending the course and highlight the importance of development new relationships and engaging with staff.

Describes how Bedfordshire PCT used the case of a fictitious patient was used to help it rethink how it delivered out of hours care. The case of 'Ethel', a fictitious 82-year-old who likes a drink or two, stumbles out of bed and falls, was used to test how older people, and particularly those who have falls access the service. It resulted in the development of '1call', which provides a single access number to provide an initial assessment.
