This report summarises the findings from a Serious Case Review after Child 1 suffered serious head injuries in November 2006, when he was only 38 days old. As a result of these injuries, Child 1 will probably be permanently disabled, with sight and hearing problems. The review considers the professional involvement with Child 1, to identify and recommend any relevant changes to professional practice, and the ways in which the different
agencies work together to safeguard children and young people. The report outlines the case review process, provides brief details of the case, key findings and recommendations. The review found nothing in the records of the agencies, or in agency reviews to suggest that the injuries could have been predicted or prevented. It raised the major issue of how to educate parents to protect babies from head injuries.
