Title:

Hospital Discharge: An evaluation of social care intervention with people who self fund their own care direct from hospital.

Abstract:

The focus for this research will look specifically at social care intervention in hospitals in North East Essex for service users whose personal finance takes them over the current defined threshold for social care funding, leaving them to self fund their own care needs on discharge from hospital. The questions for this research seek to not only look at what social cares current practice is within the North East Essex hospitals but to look at the reasons why practice intervention is delivered in such a way and what restrictions and challenges there maybe to social care workers. This research seeks to evaluate what is happening and to use any evidence to develop practice.

Aims & Objectives:

To develop current practice. To facilitate the researchers completion of the Applied Dissertation required for the MA in Advanced Social Work, University of East Anglia.
