Abstract: |
Child C was a premature baby born at 34 weeks gestation. After birth, Child C was nursed in the Special Care Baby unit, with regular visits from her mother and father. In April 2009 the Ambulance Service was called to the family home and found the baby showing no signs of life. Child C was taken to the Accident and Emergency Department where she was pronounced dead. At the inquest into the death, the Coroner returned a verdict that Child C died following a night spent sharing a bed with an adult. The cause of death remains unclear. This serious case review (SCR) examines the involvement of Agencies with the Child C and family starting with the first antenatal date and ending a week after her death. It finds that the death, and its circumstances, could not reasonably be expected to be predicted from information available to practitioners at the time. However, a number of issues have emerged through this review process. This includes the importance of recognising risk factors associated with co-sleeping with young babies. The delay in the completion of this SCR is also discussed. |
