Abstract: |
In summer 2009, an ambulance was called to the home of baby AS who was not quite 4 months old. AS was found to be in cardiac arrest and transferred to hospital. Despite intensive treatment AS did not respond and, 6 days after his admission, his life support was switched off. The case involved a number of concerning features: a CT scan showed a high attenuation area in the left frontal area thought to be blood which the parents could not explain; the Consultant Paediatrician could not rule out non-accidental injury; a month earlier AS had an emergency admission to a local hospital for apnoea and sepsis; and in January 2009 Children's Services had completed a core assessment to address the potential risk to AS’s sibling as a consequence of their father's history of sexual offending. In the light of this, the decision was made to hold a Serious Case Review. In addition, a criminal investigation was held into the circumstances surrounding the death of AS, the result of which was that the father was found guilty of manslaughter and sentenced to 4.5 years imprisonment. This executive summary provides an overview of how the Serious Case Review was conducted, a summary of events, key issues arising from the review, learning, and recommendations. |
