Abstract:

The purpose of this serious case review was to establish what lessons could be learned from the case about the way in which local professionals and organisations work individually and together, to propose action to be taken as a result, and to improve intra and inter-agency working to safeguard and promote the welfare of children. It concerns a case involving the death of a child resulting from injury and neglect. It provides a synopsis of the case and sets out identified learning points and good practice. It also makes 6 recommendations for action by Birmingham Safeguarding Children Board, Birmingham and Solihull NHS Mental Health Foundation Trust, South Birmingham Primary Care Trust, and Birmingham City Council.
