Abstract: |
This article, based on research from a facility providing intermediate care, outlines what nursing homes can learn from this service model and working with a rehabilitation team. It is about one multidisciplinary, multi-agency team's development of person-centred, intermediate care (PCIC) for older people. PCIC is available to people requiring either rehabilitation following early discharge from hospital (step down care) or as an alternative to admission to hospital (step up care). Rehabilitation, when referring to intermediate care, is highly specific for each patient but aims to promote independence by encouraging patients to learn or re-learn basic living skills. |
