This document brings together examples of good practice in end of life care. This selection of case studies was identified during the Care Quality Commission review for different groups, including visits to local areas, and are published as part of a suite of documents reporting on the CQC end of life care thematic review. The case studies are structured around a number of themes, including: providers getting it right; a strategic approach to equality-led commissioning; improvement approaches to end of life care for specific groups; coordination of care and information sharing; and community development.
